  • Machine-learning classification of bipolar disorder incorporating wearable-derived core body temperature and actigraphy-derived sleep indices

    AIM: This study aimed to classify patients with bipolar disorder (BD) and normal controls (NCs) using machine-learning models that incorporate wearable-derived core body temperature (CBT) and actigraphy-derived sleep indices. METHODS: We enrolled 23 euthymic patients with BD and 43 NCs. Sleep parameters were collected via wrist actigraphy for 14 days, and CBT was measured with a wearable device for 3 days to estimate the CBT nadir and its phase differences with sleep indices. Models were constructed with a base model using CBT- and sleep-derived features, and an extended model that included sociodemographic variables. Features were standardized (StandardScaler), and classifiers (random forest, LightGBM, and XGBoost) were evaluated. Hyperparameters were optimized using cross-validation within the training data. Performance was evaluated using repeated nested cross-validation. SHapley Additive exPlanations (SHAP) values were computed in the extended model to quantify relative feature contributions to the model output. RESULTS: In nested cross-validation at the MaxF1 operating point, the mean area under the receiver operating characteristic curve (ROC-AUC) was 0.771 ± 0.162 for the base model and 0.930 ± 0.050 for the extended model. In the extended model, SHAP suggested that total sleep time, wake time, and the wake time-CBT nadir phase difference were features potentially associated with the model output. CONCLUSION: In this small-sample study, a classification approach combining wearable-derived CBT indices and actigraphy-based sleep parameters suggested preliminary discrimination between BD and NC. Further validation is warranted in larger cohorts with balanced background characteristics, including disorders requiring differential diagnosis.

  • Higher discontinuation with dual orexin receptor antagonists than with benzodiazepines: A Japanese claims cohort study

    AIM: This study aimed to evaluate the likelihood of discontinuing dual orexin receptor antagonists in patients continuously prescribed hypnotics using a large-scale claims database in Japan. METHODS: The cohort comprised 4422 patients identified from insurance claims data, who were newly prescribed a single hypnotic with more than three consecutive prescriptions over 3 months. The outcome was defined as the absence of a hypnotic prescription for 90 consecutive days after the final prescription. Kaplan-Meier and Cox proportional hazards analyses were used to compare the hazards of discontinuing hypnotics among four classes: benzodiazepines, non-benzodiazepines, dual orexin receptor antagonists, and melatonin receptor agonists, adjusting for age, sex, and the number of diagnostic categories based on the International Statistical Classification of Diseases, Tenth Revision. RESULTS: Kaplan-Meier analysis revealed significant differences among the hypnotic classes (P < 0.001), with dual orexin receptor antagonists demonstrating the highest rate of discontinuation. Multivariable Cox regression indicated a significantly higher likelihood of discontinuing dual orexin receptor antagonists than that of benzodiazepines (hazard ratio = 1.40; P < 0.001). Older age (hazard ratio = 0.995; P < 0.001) and a higher number of International Statistical Classification of Diseases, Tenth Revision diagnostic categories (hazard ratio = 0.975; P < 0.001) were significantly associated with a lower likelihood of discontinuation. CONCLUSION: Dual orexin receptor antagonists are more likely to be discontinued than benzodiazepines after continuous hypnotic prescription, highlighting their potential clinical utility in insomnia treatment.

    AIM: A delayed sleep-wake rhythm is a common but often under-recognized characteristic of bipolar disorder (BD). This study aimed to examine the association between delayed sleep-wake rhythm and functional impairment using actigraphy. METHODS: Participants with BD in clinical remission (N = 47) were recruited. Sleep parameters, including the midpoint of sleep (o'clock), total sleep time (minutes), sleep efficiency (%), and wake after sleep onset (minutes), were averaged over a 14-day period using continuous actigraphy. Functional impairment was assessed using the 12-item World Health Organization Disability Assessment Schedule 2.0 (WHODAS 2.0). Correlation analyses were conducted to examine associations between total WHODAS 2.0 scores and sleep parameters. Multiple regression analyses were performed with WHODAS 2.0 total scores as the dependent variable and sleep parameters as independent variables, controlling for mood symptoms and insomnia severity. RESULTS: Correlation analysis revealed a significant positive relationship between the midpoint of sleep and total WHODAS 2.0 scores (r = 0.424, p = 0.003). Multiple regression analysis identified the midpoint of sleep as a significant predictor of total WHODAS 2.0 scores (β = 0.488, p = 0.005). CONCLUSION: Delayed sleep-wake rhythms may contribute to functional impairment in euthymic individuals with BD. These findings suggest that advancing sleep-wake rhythm may contribute to improvements in social functioning. Actigraphy holds potential as a digital biomarker for assessing functional outcomes in this population.

    Accurately assessing and predicting suicidal behavior in patients with depression are challenging for researchers and clinicians. We examined various risk factors for suicidal behavior during major depressive episodes (MDE), especially focusing on depressive mixed state (DMX) and anxious distress (AD). We recruited 187 patients with MDE and divided them into two groups-with and without suicidal behavior-defined as the cut-off score of 1 or more on the suicidal behavior sub-item in the quick inventory of depressive symptomatology-self report. The presence of DMX was defined as a total score of 13 or more on the self-administered 8-item questionnaire for DMX. We used multivariate logistic regression analysis with the presence or absence of suicidal behavior as a dependent variable for investigating factors associated with suicidal behavior. The with suicidal behavior group was younger and indicated a greater proportion of past suicide attempts, AD, and DMX than the without suicidal behavior group. Logistic regression analysis revealed that AD (P = 0.020) and DMX (P = 0.018) were significantly associated with suicidal behavior. AD and DMX may promote suicidal behavior during MDE. These two psychopathological features should be carefully monitored and intensively treated for the prevention of suicide-related events.
