    <p>冠動脈大動脈起始異常症は比較的稀な先天性冠動脈異常である．心筋虚血や心室性不整脈が問題となるが，初発症状が心停止である例が約半数との報告もある．しかし，かかる病態に対する手術適応や手術術式に関して不明な点も多い．今回，右冠動脈大動脈起始異常症に対して外科治療を行い良好な結果を得たので報告する．繰り返す胸部圧迫感を主訴とする47歳男性に精査を行ったところ，右冠動脈が左バルサルバ洞より分岐する右冠動脈大動脈起始異常症であった．血液検査，心電図，心臓カテーテル検査を含め客観的な心筋虚血所見を認めなかったものの，右冠動脈の比較的急峻な大動脈からの起始角度，両大血管間に挟まれた走行形態が胸部症状に関与している可能性と，突然死の可能性が否定できなかったため，手術の方針とした．手術は右冠動脈移植術を施行した．画像上良好な結果が得られ，術後一年の現在，胸部症状の再燃なく外来経過観察中である．</p>
