    <p>本年2月より新しい塞栓物質としてShape memory polymerを用いたIMPEDEが認可されたため,その初期経験を報告する。症例は82歳男性。右内腸骨動脈瘤に対してEVARに先がけIMPEDE(IMP-10)を用いて上殿動脈・下殿動脈をそれぞれ塞栓した。血栓形成遅延を予防するため,塞栓確認造影のタイミング・回数を検討する必要があるが,術後CTではアーチファクトが少なく,エンドリークの評価がしやすいことから,IMPEDEは今後期待される塞栓物質である。</p>

    <p>Deep hypothermia in thoracoabdominal aortic aneurysm operations is considered extremely useful for ensuring sufficient time to reconstruct the segmental arteries feeding the spinal cord. However, because the amplitude of motor evoked potentials (MEPs) decrease or disappear during deep hypothermia, feasible methods for assessing spinal cord circulation have not yet been reported. Performing additional segmental arterial reconstructions that rely on MEPs is also impractical. In the present case, to ascertain spinal cord circulation under deep hypothermia, we intraoperatively measured the reconstructed segmental arterial pressure in real time and investigated whether sufficient spinal cord blood flow had been attained.</p>

    <p>原発性心臓腫瘍は稀な疾患であり,その発症率は剖検例の0.0017~0.33%とされているが,心臓超音波検査の進歩および普及による診断学の発展に伴い,その診断率は年々上昇している.乳頭状線維弾性腫は粘液腫についで多い心臓原発性良性腫瘍で,多くは大動脈弁および僧帽弁に単発で生じることが多い無血管性乳頭腫であり,4弁すべてに発生した症例報告はない.今回われわれは意識消失を契機に発見された,若年女性の4弁すべてに発生した乳頭状線維弾性腫の1例を経験したので文献的考察を加え報告する.</p>
