  <p>A 59-year-old Japanese woman was referred for an extremely low level of circulating high-density lipoprotein cholesterol (HDL-C). The serum HDL-C level had long been within the normal range but suddenly decreased asymptomatically to 7 mg/dL. She had no typical symptoms associated with familial lecithin, cholesterol acyltransferase deficiency (FLD), including proteinuria, anemia, and corneal opacity. The circulating level of ApoA-1 was also markedly decreased at 48 mg/dL, and the proportion of esterified cholesterol to free cholesterol was irregularly low at 26%. Whole-genome sequencing revealed no apparent pathological mutations in the <i>LCAT</i> gene. Notably, anti-LCAT antibodies were detected in the serum at 146±1.7 ng/mL, resulting in her being diagnosed with acquired LCAT insufficiency (ALCATI) caused by anti-LCAT antibodies. Five years after her HDL-C levels spontaneously decreased, they increased without any identifiable cause. To our knowledge, only six cases of ALCATI caused by anti-LCAT antibodies have been reported to date. In contrast to the present case, previously reported cases of ALCATI manifested proteinuria that improved with steroid therapy. The unique clinical course in the present case highlights the heterogeneity of ALCATI, warranting further research to clarify the molecular pathophysiology of FLD and ALCATI.</p>

  日本では、アルドステロン血漿中濃度(PAC)の測定法として放射標識免疫測定法(RIA)が従来用いられてきた。しかし、本法には様々な問題があるため、2021年4月から化学発光酵素免疫測定法(CLEIA)が導入され、日本内分泌学会は原発性アルドステロン症(PA)の新しいガイドラインを発表した。本研究では、RIAからCLEIAへの移行がPA診断に及ぼす影響を評価した。2012年4月～2021年3月にPAと診断され当院内科に入院した患者190例(平均51±12歳、男性36.8%)のデータを分析した。変換式を用いてRIAで測定したPAC(PAC(RIA))をCLEIAで測定した場合の推定PAC(ePAC(CLEIA))に変換した。スクリーニング(SC)、カプトプリル負荷試験(CCT)、生理食塩水注入試験(SIT)に基づき、旧ガイドラインに従いPAC(RIA)から判断した場合と新ガイドラインに従いePAC(CLEIA)から判断した場合の診断の不一致率を評価した。旧ガイドラインでPAと診断されず新ガイドラインでPAと診断された患者、および旧ガイドラインでPAと診断され新ガイドラインでPA陰性とされた患者を不一致例とした。その結果、診断不一致率はSCで6.4%、CCTで10.1%、SITでは不一致は認められなかった。PAC(RIA)を用いた旧ガイドラインによる診断とePAC(CLEIA)を用いた新ガイドラインによる診断の不一致率は3.7%であった。以上より、旧ガイドラインでPAとされた患者の一定数は新ガイドラインで異なる診断を受けることが示された。

  症例は34歳女性で、建物の4階から飛び降り、その2日後に当院に搬送された。不全四肢麻痺がみられ、MRIでC7椎体の脱臼骨折とT1の軽度後方転位が明らかになった。明白な内臓損傷は観察されなかった。外傷後5日目に待機的後方脊椎固定術(C3～T4)を施行した。入院後7日目に血液検査は高カリウム血症(カリウム6.7mEq/L)と低ナトリウム血症(ナトリウム133mEq/L)を示した。好酸球増加症、貧血、肝酵素とプロラクチンの血清レベルの上昇、性腺機能低下症、甲状腺機能低下症、続発性副腎機能不全、低ソマトメジンCレベルが明らかになった。アルギニンバソプレシンの血漿レベルは測定可能範囲未満であった。飛び降りた日から6週間後のMRI所見は下垂体茎離断症候群の特徴と一致した。患者はヒドロコルチゾンとレボチロキシンナトリウム水和物による治療を開始した。月経は転落後に初めて止まり、200日後も再開しなかった。

  糖尿病治療の中断と糖尿病性網膜症(DR)の発症および進展の関連について検討した。眼底検査で増殖性DRを認めない2型糖尿病患者417名(DRなし281名、非増殖性DR 136名)を治療中断歴(12ヵ月以上の通院中断)の有無で二分し、検査後4年以降のDRの新規発症率と進展率を比較した。治療中断歴あり群(53名、13%)はなし群より若年であった。追跡期間中央値7年のDR発症率は25%(69/281)、進展率は30%(41/136)、発症/進展(いずれかの発生)率は26%(110/417)であった。治療中断歴あり群はなし群に比べDR発症率(58% vs 20%)、進展率(40% vs 28%)、および発症/進展率(51% vs 23%)が高かった。治療中断はDRの発症(多変量調整オッズ比4.20)および発症/進展(2.70)と関連を示した(P<0.01)。糖尿病治療の中断はDR危険因子である可能性が示唆された。
