    This study aimed to compare elbow alignment among common elbow fracture types in children. This retrospective study included 288 patients who were diagnosed as having supracondylar fracture (n=172), lateral condylar fracture (n=93), or radial neck fracture (n=23) in children younger than 16 years of age. We investigated the radiographic elbow parameters of the unaffected side; the carrying angle, humerus-elbow-wrist angle, and humerocondylar angle. To evaluate the independent influence of elbow alignment for different fracture types, multiple logistic regression analysis was used. Univariate analysis showed that the carrying angle and humerus-elbow-wrist angle were significantly smaller in lateral condylar fracture than in the other fractures, however the humerocondylar angle showed no significant difference between the fractures. Multiple logistic regression analysis showed that a smaller carrying angle than the mean value of the supracondylar fracture group (15.4°) tended to be associated with lateral condylar fracture, the humerus-elbow-wrist angle showed the same result (< 12.8°) as the carrying angle. It was found that a larger carrying angle tended to be associated with radial neck fracture, although the humerus-elbow-wrist angle had no significant influence on the fracture type. Understanding the association between elbow coronal alignment, especially the carrying angle, and elbow fracture types may prevent surgeons from overlooking fractures. However, sagittal alignment was not significantly different between the fracture groups. The result that the humerus-elbow-wrist angle had no significant influence might have been caused by alignment of humerus and proximal ulna, not forearm. Ryukyu Med. J., 42 (1~4) 1~10, 2023
