Impact of Consultation Length on Satisfaction in Patients with Chronic Low Back Pain: A Nationwide Multicenter Study in Japan.

<p>Introduction: Chronic low back pain (CLBP) is a major health burden worldwide and requires patient satisfaction with treatment. Consultation length can be an important factor in patient satisfaction, but few studies have investigated the impact of consultation length on satisfaction in patients with CLBP. This study tried to elucidate the impact of consultation length on clinical outcomes in patients with CLBP.</p><p>Methods: This study is part of an analysis using the database of the nationwide, multicenter cohort for CLBP performed by the Project Committee of the Japanese Society for Spine Surgery and Related Research. A total of 427 patients aged 20-85 years (median age, 73.0 years; female, 58.6%) with CLBP were prospectively followed-up monthly for 6 months. Multivariable nonlinear regression analyses were performed to assess the effect of consultation length on outcome measures including subjective satisfaction score, EuroQol 5-dimension, Japanese Orthopaedic Association (JOA) score, Roland-Morris Disability Questionnaire, JOA Back Pain Evaluation Questionnaire, visual analog scale (VAS) and Medical Outcome Survey short-form 8-item health survey that evaluated at the next phase. Furthermore, we assessed whether the effect of consultation length on patient satisfaction was modified by the baseline Brief Scale for Psychiatric Problems in Orthopaedic Patients (BS-POP) score for patient and physician versions.</p><p>Results: VAS for CLBP was the only score that correlated significantly with consultation length (P = 0.018). Satisfaction score showed a significant positive correlation with consultation length in patients with the highest baseline BS-POP scores (P < 0.2). Moreover, consultation lengths more than 7.6 min and 15.1 min offered increase of satisfaction if patients show the highest BS-POP scores on patient and physician versions, respectively.</p><p>Conclusions: These findings suggest that a sufficiently long consultation is an important factor for subjective satisfaction in the patients with CLBP, particularly in patients with psychological problems.</p>

上腕二頭筋腱遠位皮下断裂の治療経験

上腕二頭筋腱遠位皮下断裂は稀な外傷で，保存療法では肘屈曲力と前腕回外力が低下するため手術療法が推奨される．【対象と方法】症例は4例の男性で，年齢は平均54歳であった．手術は2-incision法にて施行し術後観察期間は平均7か月であった．手術待機期間， MRI矢状断画像での上腕骨小頭遠位端から腱断端までの距離，一次縫着の可否，最終観察時の肘関節可動域と筋力，術後合併症を調査した．【結果】待機期間は平均21.2日，腱短縮距離は平均6.2cmであった．3例は一次縫着が可能であったが ，1例に腱移植を用いた再建術を施行した．全例において肘関節可動域と屈曲力は健側と同等であった．前腕回外力はMMT4であったが日常生活や仕事に支障はなかった．1例に後骨間神経麻痺を認めたが完全回復した．【まとめ】2-incisionでの修復術，再建術は共に短期成績は良好であった．

【はじめに】小児内反肘変形に対するカスタムメイドガイドは，健側CT鏡像から作成されたデバイスを用いることで三次元の矯正が可能であり，手技も簡易化する．当科では2019年1月より本法を施行しており，従来法と比較した．<BR>【方法】対象は 6例 6肘．カスタムメイドガイド使用群（Guide群） 3肘，従来法群（C群） 3肘．両群間における手術時間，出血量，術直後の矯正角度，術後合併症を比較，検討した．<BR>【結果】平均手術時間は短い傾向にあった．出血量，術直後矯正角度は同等であった．術後合併症は Guide群で K鋼線 back out を 1 肘，C群は一過性尺骨神経麻痺，lateral prominence を各 1 肘に認めた．<BR>【まとめ】本法を導入したばかりだが従来法と比べて手術時間が短い傾向にあった．

外傷歴の無い内反肘変形に伴う後外側回旋不安定症（PLRI）の治療報告は少ない．両側の非外傷性内反肘で右側にPLRIを認める症例に対し，上腕骨矯正骨切りと靭帯再建術を行った．【症例】14歳男性．外傷歴なく両肘内反変形が出現．肘関節の亜脱臼，自己整復を繰り返していた．左側の亜脱臼は12歳ごろ自然に消失したが，右側は持続し，痛みを認めたため手術を予定した．肘内反角は右14 ° 左5° ，Humerus-Elbow-Wrist angleは右-13° ，左-5° であった．手術では右上腕骨に対し14° の外反矯正骨切り術を行い，長掌筋腱を用いて外側尺側側副靭帯を再建した．術後6か月で不安定性を認めなかった．【考察】本症例は先天性の内反変形が外側支持機構の弛緩を生じPLRIを生じたと考えられた．本症例では靭帯再建術だけでなく上腕骨矯正骨切り術を併用したことで関節不安定性が改善されたと思われた．

RA 肘への Kudo-type5TEA 術後に上腕コンポーネントのゆるみが原因で，顆部の骨欠損を再建，尺側コンポーネントを温存して再置換術施行．【症例 1】 73 歳女性． Xp像でコンポーネントの前方偏位，内側顆欠損．内側顆を bi-cortical 腸骨で再建し，前方偏位部に皮質骨，海綿骨を充填して骨セメント設置．【症例 2】 66 歳女性． Xp像で上腕骨前方の穿孔，約 14mm の沈み込み．顆部中央に約 3.5 ｘ 4.0cm 骨欠損．採型した bi-cortical 腸骨の髄腔内にステムを通し，コンポーネント頚部内に噛み込ませて顆部中央に設置した．【症例 3】 64 歳女性． Xp 像で上腕骨前方の穿孔，約11mm の沈み込みあり．顆部中央に約 3.5 ｘ 3.5cm の骨欠損を認めた．症例 2 と同様に再置換術を施行した．短期であるが経過は良好である．

OBJECTIVE: The mammalian target of rapamycin (mTOR) is a serine/threonine kinase that integrates nutrients to execute cell growth. We hypothesized that mTOR is influential in the intervertebral disc-largest avascular, low-nutrient organ. Our objective was to identify the optimal mTOR inhibitor for treating human degenerative disc disease. DESIGN: mTOR complex 1 (mTORC1) regulates p70/ribosomal S6 kinase (p70/S6K), negatively regulates autophagy, and is controlled by Akt. Akt is controlled by phosphatidylinositol 3-kinase (PI3K) and mTOR complex 2 (mTORC2). mTORC1 inhibitors-rapamycin, temsirolimus, everolimus, and curcumin, mTORC1&mTORC2 inhibitor-INK-128, PI3K&mTOR inhibitor-NVP-BEZ235, and Akt inhibitor-MK-2206-were applied to human disc nucleus pulposus (NP) cells. mTOR signaling, autophagy, apoptosis, senescence, and matrix metabolism were evaluated. RESULTS: mTORC1 inhibitors decreased p70/S6K but increased Akt phosphorylation, promoted autophagy with light chain 3 (LC3)-II increases and p62/sequestosome 1 (p62/SQSTM1) decreases, and suppressed pro-inflammatory interleukin-1 beta (IL-1β)-induced apoptotic terminal deoxynucleotidyl transferase dUTP nick end labeling (TUNEL) positivity (versus rapamycin, 95% confidence interval (CI) -0.431 to -0.194; temsirolimus, 95% CI -0.529 to -0.292; everolimus, 95% CI -0.477 to -0.241; curcumin, 95% CI -0.248 to -0.011) and poly (ADP-ribose) polymerase (PARP) and caspase-9 cleavage, senescent senescence-associated beta-galactosidase (SA-β-gal) positivity (versus rapamycin, 95% CI -0.437 to -0.230; temsirolimus, 95% CI -0.534 to -0.327; everolimus, 95% CI -0.485 to -0.278; curcumin, 95% CI -0.210 to -0.003) and p16/INK4A expression, and catabolic matrix metalloproteinase (MMP) release and activation. Meanwhile, dual mTOR inhibitors decreased p70/S6K and Akt phosphorylation without enhanced autophagy and suppressed apoptosis, senescence, and matrix catabolism. MK-2206 counteracted protective effects of temsirolimus. Additional disc-tissue analysis found relevance of mTOR signaling to degeneration grades. CONCLUSION: mTORC1 inhibitors-notably temsirolimus with an improved water solubility-but not dual mTOR inhibitors protect against inflammation-induced apoptosis, senescence, and matrix catabolism in human disc cells, which depends on Akt and autophagy induction.
